Website Privacy & HIPAA Notice
Effective Date: June 23, 2026. We may update this notice at any time. The current version is always available at this URL. Material changes will be noted with a revised effective date.
Hillview Dental Group respects your privacy and protects patient information in accordance with applicable health privacy laws. This notice summarizes how we handle information collected through this website and how patient health information may be used in connection with dental care.
Information we may collect
When you submit a website form, call us, or email us, we may collect your name, contact details, appointment preferences, and any information you choose to share. Our website may also collect limited technical information, such as browser type, device information, and general usage data, to help maintain site performance and security.
Protected health information
As a dental practice, we may maintain Protected Health Information (PHI) about patients. PHI may be used or disclosed for treatment, payment, and health care operations, such as coordinating dental care, submitting insurance claims, confirming appointments, training staff, quality review, and operating the practice. We may also disclose information when required by law, to business associates that support our practice under appropriate privacy obligations, or when necessary to address a serious threat to health or safety.
Business associates and service providers
We may work with third-party service providers that support our website, communications, insurance, billing, and practice operations. These may include WP Engine for website hosting and infrastructure, Paubox for secure online form transmission, DentalXchange for insurance verification or claims-related services, dental practice management or messaging software providers for appointment communications, and other billing, IT, analytics, or administrative support providers. When a vendor handles Protected Health Information on our behalf, we expect appropriate privacy and security obligations, including a Business Associate Agreement when required by HIPAA.
Your health information rights
Patients may have the right to request access to records, ask for corrections, request certain restrictions, request confidential communications, ask for an accounting of certain disclosures, receive a paper copy of this notice, and receive notification if unsecured health information is breached. To exercise these rights, contact our office. California patients may also have additional rights under the California Confidentiality of Medical Information Act (CMIA), Civil Code §56 et seq.
Website forms and Paubox
Our online contact form is provided through Paubox. Appointment requests and inquiries submitted through this website are transmitted to our office through Paubox rather than standard website email. Website forms are intended for appointment requests and general communication. Please do not use website forms for emergencies. For urgent dental concerns, call 510-651-1440. If you are experiencing a medical emergency such as difficulty breathing or severe swelling, call 911 immediately.
How we use website information
Information submitted through this website may be used to respond to your inquiry, schedule or confirm appointments, communicate with you about dental care, and improve our website. We do not sell personal information to third parties.
Cookies, analytics, advertising, and call tracking
Our website may use cookies and similar tools for site functionality, analytics, advertising measurement, and call tracking. These tools may include Google Analytics, Google Ads, and call tracking services that help us understand website traffic, advertising performance, and whether calls came from the website. These tools are not intended to collect Protected Health Information. You can control cookies through your browser settings and may use Google's available ad and analytics controls.
Third-party links and financing partners
This site may link to third-party websites, including map services, insurance resources, review platforms, and patient financing providers such as CareCredit and Cherry. When you leave our website, those third parties control their own privacy and security practices. Please review their policies before submitting personal or financial information.
Text messages and email
If you provide a mobile number or email address, we may use it to communicate about appointments and care coordination. Standard email and SMS may not be fully secure communication channels. Do not send sensitive clinical details by standard email or text unless directed by our office through an approved secure method.
Complaints and questions
If you believe your privacy rights have been violated, you may contact our office. You may also file a HIPAA complaint with the U.S. Department of Health & Human Services Office for Civil Rights. We will not retaliate against anyone for filing a complaint in good faith.
